Job Summary and Qualifications

STRUCTUAL HEART PROGRAM PATIENT CARE NAVIGATOR - WEEKDAYS ONLY(M-F 8 hours) and must have Cardiac or CVOR experience

  • Position Summary:The Registered Nurse - Structural Heart Program Coordinator, Blake Structural Heart Clinic, functions under the direction of the manager and physician leaders of the Structural Heart Program, Administrative Director/Vice President of Cardiovascular Services and the West Florida Division Structural Heart Program Leader. The position is responsible for coordinating patient care and evaluation, align resources to support program initiatives, program development, develop and maintain relationships with internal and external referring physicians, and development of a program database to include but not limited to following procedures: Transcatheter Aortic Valve Replacement (TAVR), Percutaneous Left Atrial Appendage Closure (LAAC), Transcatheter Mitral Valve Repair/Replacement (TMVR).The Registered Nurse Coordinator, Your Hospital Name Structural Heart Clinic, supports the mission, vision, and values and must demonstrate a commitment to assist with staff, patient and community education, promote and maintain quality service, research, and program outcomes.Summary of Functions and Responsibilities:1. Implements and ensures best practices for all clinic protocols, practices, procedures and policies’ meets all regulatory and compliance guidelines, ensuring a safe environment for all patients.2. Serves as a clinical resource for structural heart/valve patients; coordinates diagnostic studies and/or clinical interventions when appropriate, as well as measures patient-specific and aggregate outcomes; tracks new technology trends, research findings, treatment models, etc.3. In collaboration with medical leadership and advanced practice providers, incorporates new technologies, treatments and research into clinical practice. Tracks and analyzes physicians’ practices, referral patterns and market data; recommends initiatives in response to community need and growth opportunities.4. Works with physician leader(s) and advanced practice provider to prepare clinical in-services, CME courses, marketing and collateral materials to create program awareness and understanding. Routinely reviews and ensures patient and referring physician education and collateral materials are current, coordinated, available and effective.5. Works with physician leaders, advanced practice provider, the Administrative Director/Vice President, Cardiac Services and the administrator for the Structural Heart Center to identify and recognize partnership(s) and growth opportunities. Interfaces with industry representative(s) for program awareness, research and partnership opportunities. Works with physician leaders for program content and structure. Builds internal and external program awareness, development and growth.6. Facilitates referrals for structural heart procedures as required, specifically transcatheter valve and LAAC procedures. Coordinates appropriate testing, insurance authorizations and collates results for review. Utilizes appropriate software for coordination of images/studies (ValvePoint, CoreValve Connect or similar).7. Assists the physicians and advanced practice providers in facilitating the pre-operative evaluation, including history and physical examination, assisting with ordering appropriate diagnostic testing. Performs duties related to admission, including facilitating pre-operative orders, patient documentation, appropriate testing and scheduling of procedures.8. Assists in post-operative care in the intensive care and step-down units as requested within the scope of the RN licensure.9. Assists in post-discharge short and long term follow-up care in clinic, including patient assessment, facilitating management of care, and coordination/ordering of follow-up testing/procedures, etc, that is within the scope of the RN licensure.10. Facilitates follow up examinations and evaluations for prospective patients as well as follow-up care in the Structural Heart Clinic per regulatory requirements.11. Coordinates valve conference and prepares patients’ case presentations. Creates a tracking system/database to follow patients. Schedules and coordinates structural heart procedures.12. Utilizes Structural Heart Dashboard (CVTrac) to identify patients with valvular heart disease, potential surgical and transcatheter based procedures candidates. Ensures all testing, treatment and data collected on the patient is entered in the dashboard.13. Maintains database consistent with the recommendations and guidelines set forth by professional organizations and CMS; provides outcome reports from the respective registries to the administrative team.14. Performs other duties as assigned.Position Requirements:

a. Licensure/Certification/Registration: State of Florida Registered Nurse license; BLS Healthcare provider; ACLS certification

a. Education: A graduate of NLN School of nursing with Registered Nurse license in Florida.

a. Experience: A minimum of two years’ experience with one year experience in cardiology, preferably heart failure, valvular disease or critical care preferred.Performs in accordance with delineated job responsibilities and satisfactorily completes a yearly evaluation based on these responsibilities. Must possess the knowledge and skills necessary to effectively meet the needs of the patient/internal and external customer.
