EVERGREEN ELM INC Direct support professionals (Residential) in Bradford, Pennsylvania

This employee will live-in and/or work in one of our group homes serving individuals with intellectual disabilities, assisting the Chief Executive Officer in carrying out the objectives of the program as outlined by 55 PA Code Chapter 6400 and Evergreen Elm Policy & Procedures. Employee will assist the individuals in learning to live and function as independently in the home and community.

Duties; Guidelines:

  • Will establish and maintain rapport with the individuals, co-workers and families of the individuals.
  • Will participate in the development, implementation and documentation of Plan of Care. This includes assessment.
  • Will directly assist the individuals in areas of skill development requiring specialized individual attention.
  • Will provide a normal, home-like setting to teach daily living skills.
  • Will counsel, educate and praise the individuals to deal more effectively with personal problems, values and responsibilities.
  • Will set a good example of behavior through everyday mature, responsible role modeling.
  • Will initiate and implement recreational, social, educational and therapeutic activities within the home and community designed to meet the individual needs of the individuals in learning to become as self-sufficient as possible.
  • Will administer and monitor the individuals' medication.
  • Will assist individuals with personal hygiene.
  • Will assist in arranging specialized services for the individuals (i.e. doctor/dental appointments, speech therapy, physical therapy, etc.)
  • Will maintain the home and surroundings in a neat, orderly manner and assist the individuals with necessary home maintenance. This includes thorough housekeeping (cleaning, laundry, garbage, meal prep, shopping) skills and outdoor clean-up (snow removal, mow lawn).
  • Will ensure the safety of the Agency vehicle. This includes maintenance and regular cleaning.
  • Will prepare accurate documentation in all areas as required by State Regulations and Agency policy including case notes, daily logs, staffing notes, medication logs, van reports, etc.
  • Will respond to emergency crises to protect the health and safety of the individuals.
  • Will assist individuals to evacuate homes in the time frame set by the local fire department.
  • Will transport the individuals to appointments, activities, workshops, etc. as necessary.
  • Will participate in staffing, case conferences and training sessions as necessary.
  • Will perform related work as required by the R. P. Director I and the Chief Executive Officer.

You will receive direct supervision from the Residential Program Supervisor on a daily basis. The R. P. Supervisor is on call to handle any problems which may occur when he or she isn't working. If the R. P. Supervisor isn't available, you report directly to the R. P. Director I. In house staff meetings are held to review and assign work and to discuss any problems that need attention. The R. P. Supervisor reviews your work informally on a daily basis as needed. The R. P. Supervisor reviews your work formally after six months and then once a year with a written evaluation of your job performance.
