State of Massachusetts Program Coordinator, Money Follows the Person in Boston, Massachusetts

The Executive Office of Elder Affairs (EOEA) is seeking a proactive, mission-driven and dynamic individual to fill the EOEAProgram Coordinator, Money Follows the Personposition. The Money Follows the Person (MFP) Demonstration is a federally funded program that helps older adults and people with disabilities move from nursing facilities, chronic disease or rehabilitation hospitals, or other qualified facilities back to the community. The successful candidate must possess program management experience and the ability to communicate clearly in order to convey and articulate information in writing and orally in a clear and meaningful manner to EOEA managers, senior staff, the MFP Project Office, and external stakeholders. The Program Coordinator is responsible for tracking and managing MFP Demonstration enrolled participants and will work closely with program staff and outreaching and responding to stakeholders. The Program Coordinator works collaboratively across Health and Human Services agencies. This position will be located within the Executive Office of Elder Affairs, Home Care Unit. Reporting to the Director of Home Care. The incumbent will be responsible for coordinating and monitoring the MFP Demonstration activities conducted by the Aging Services Access Points (ASAPs) to ensure effective operations, compliance, data sharing, and reporting. This individual will be responsible for coordinating with program staff and team members on nursing facility discharges and reviewing and analyzing participant and prospective participant records, including reviewing data related to the MFP Demonstration to determine progress and effectiveness. The incumbent will support critical aspects of the agency’s mission by ensuring MFP Demonstration program requirements are adhered to, MFP Demo services are provided consistently, quality measurements are in compliance and reporting meets expectations as outlined. This is a 5 year grant funded position. Renewable subject to available funding. /Duties and Responsibilities (these duties are a general summary and not all inclusive):/ * Support the coordination, monitoring, and assist in development of initiatives related to the implementation, operation, and overall management of the MFP Demonstration program from the EOEA perspective. * Provide consultation, direction, and technical assistance to ASAPs in response to MFP Demonstration program participants. * Initiate action with ASAPs to ensure programmatic compliance related to regulations, sub regulatory guidance, and program instruction. * Review all MFP Demonstration program reporting including performance measures, aggregating data, preparation of data reports, and presentation of findings. * Conduct education and training on MFP Demonstration program, program requirements and changes. * Coordinate and support interagency and cross agency regular collaboration related to the MFP Demonstration Program. * Support the coordination and facilitation of interagency and cross agency governance of the MFP Demonstration Program. * Advise the Director of Home Care, the Clinical Nurse Manager, Community Transition Liaison Manager and the MFP Demonstration Project Office on program issues related to the MFP Demonstration program. * Work with the MFP Demonstration Project Office to review, or amend necessary documents, trainings, contracts, and policies. * Act as the primary EOEA contact for the following: o Technical Assistance related to the operation and management of the MFP Demonstration Program within Health and Human Service agencies o MFP Demonstration program implementation, operation, management, and troubleshooting at EOEA. o Liaison with other human service agencies and organizations on all interagency collaborations related to MFP Demonstration. * Assist with the following projects and tasks: o Development and implementation of standards to monitor and evaluate MFP Demonstration activities as they relate to Frail Elder Waiver, Home Care Program, Community Transition Liaison Program, and Aging Services Access Points (ASAPs), with a goal of ensuring equitable and high-quality service delivery. o Data collection, reporting, tracking, trend identification, compliance monitoring and remediation for MFP Demonstration and ASAP involvement. o Response to time sensitive requests for information, and public document requests. o Management of participant records within database and software specific to the MFP Demonstration program. o Cross integration management of consumer records within EOEA cloud-based database. * Other duties, as assigned. * Preferred Qualifications:* * Knowledge of: o Support services network, including programs and services offered by the Executive Office of Elder Affairs, Massachusetts Rehabilitation Commission, Department of Mental Health, Department of Developmental Services, MassHealth, Home and Community Based Waiver programs, and other Health and Human Service organizations. o Long-term care industry and/or, health and home care industry preferred, Massachusetts Housing programs and options preferred. o Data collection, analysis, trend identification and remediation * Ability to: o Interact tactfully and effectively with others. o Exercise discretion in handling confidential information. o Identify problems as they arise and recommend appropriate response. o Manage up and proactively check in with supervisor to communicate progress, ask clarifying questions, and align on next steps. o Problem solve and think critically, with very strong attention to detail. o Conduct detailed review of participant records and provide related analysis. o Build relationships and complete assigned tasks with a strong customer service orientation, as well as strong teamwork and collaboration skills. o Contribute and achieve results in a multidisciplinary team environment _Agency Mission:_* * By the end of this decade, one in five people living in Massachusetts will be 65 years or older. The Executive Office of Elder Affairs (EOEA) serves 1.7 million older adults in the Commonwealth through 350 Councils on Aging and 24 regional Aging Services Access Points. EOEA is responsible for ensuring that the services provided are of the highest quality, are consumer-focused, and are delivered cost-effectively. EOEA oversees a budget nearing $750 million from state and federal sources that supports program implementation through a regional distribution network of 24 Aging Services Access Points (ASAPs) and 350 Councils on Aging (COAs) nested within the 351 municipalities of the Commonwealth. EOEA is responsible for managing the statewide Aging Services Access Points network, the Home Care Program, Adult Protective Services (APS), the certification of Assisted Living Residences (ALRs), and is also responsible for numerous other state and federal programs including Prescription Advantage, home-delivered and congregate meal programs, the SHINE program, which provides counselling to older people regarding health insurance options, congregate and supportive housing, the Family Caregiver Support Program, and SCSEP (a program for older job seekers). EOEA also leads and staffs key public bodies and commissions pertaining to older adults including the Alzheimer’s Advisory Council, the Governor’s Council to Address Aging in MA, and the Assisted Living Advisory Council.
