Direct Support Professional - Day Habilitation Trainer
Full Time, Monday-Friday, 1st Shift, Wage $16.00-17.25/hr

The Day Habilitation Program is about promoting independence and support for individuals with moderate to severe disabilities in the Human Services field. In addition to the activities that take place inside our facilities, you’ll also find us:

Volunteering for local organizations
These are a few of the things a Direct Support Professional (DSP) Day Habilitation Trainer will do:
Implement goal plans and data collection to monitor progress.
Transport individuals to volunteer and recreational activities in the Community. Also provide the necessary supervision, training and support during these outings.
Maintain accurate daily participant records, charts, logs, and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies.
Maintain an orderly, clean, safe, environment at all times.
Order supplies for with approval from the Direct Supervisor.
Assist an individual with toileting, if needed.
Complete monthly progress notes, semi-annual and annual reviews as assigned by or in the absence of the Direct Supervisor.
Assist in boarding vehicles and disembarking from vehicles.
Monitor passengers and maintain orderliness on vehicle to ensure passenger safety and adherence to safety regulations.
Assist in planning activities which will foster community integration.

Requirements
High School Diploma or GED
Possess a valid driver’s license acceptable to the agency’s insurance carrier
Salary Description
$16.00-$17.25
