Description:

Assesses nutrition status of assigned patients and implement all components of the nutrition care process, according to food and nutrition department policies and local regulatory standards (IDPH, TJC, CMS, etc.). Monitors and evaluates nutrition interventions, makes appropriate adjustments to plans, as needed, and reports results of quality outcomes in the inpatient and outpatient setting.

Assess, interpret, plan, monitor and document the nutritional status and nutritional needs of patients using evidence-based standards of care, policies and practice guidelines. Perform a nutrition-focused physical exam on patients. Identify and provide age-specific nutrition care to meet the cultural needs of the patients and their families. Work within an inter-disciplinary team to provide a plan of care to achieve the patient's medical goals.

Educate patients, care givers, students and other health care colleagues on the effect of therapeutic diets and nutrition support to maintain/improve nutritional status.

When assigned, provide support and supervision to the daily infant formula preparation routine, which includes identifying and calculating recipes to individualize the formula to meet the nutritional needs of the infant.

Participate in department activities to enhance patient satisfaction.

Keep current on nutrition research and incorporate into daily practice. Other clinical duties as assigned.
