Qualifications

The Community Behavioral Support provides direct services to children and their families experiencing a behavioral, emotional, or mental health disturbance and/or issues with substance use. Services provided by this specialist will allow easy and flexible access to behavioral support services that are home and community based in a family centered approach. There is an emphasis on providing mobile services in the community to support integration within the community as well as refer to Emergency Services if needed.

MAJOR RESPONSIBILITIES:\ DIRECT CLINICAL SERVICES:Provides screening and behavioral support to children, families and/or groups with behavioral and/or emotional mental health needs. Services will be provided over the phone or in the community, which includes but is not limited to: home, school, or office as needed. Facilitates referrals and communication between fellow staff and other agencies for other services on behalf of the child. Attends team meetings to plan and monitor services. Provides short-term client consultation, collateral information, and social support as needed.

If or as assigned to provide Substance Use services, or within related SUD grants, the incumbent will show competencies in screening, assessment, engagement, treatment planning, collaboration, referral, motivational interviewing, HIPAA and 42 CFR Part 2, and other evidence-based counseling and professional ethics theories, to provide effective care. Participation in the annual 5 week long summer Camp Be A Kid program as program staff.

SERVICE COORDINATION AND SOCIAL SUPPORT:Makes community/home visits to support children and families as needed for a short-term basis to supporting continued integration in their community. Facilitates communication between fellow staff and other agencies on behalf of the child if needed. Attends team meetings to plan and monitor services. Provides case coordination and consultation with th assigned case manager, on an as needed basis. These services may include service coordination with other agencies and services; collateral contacts with other family members or significant others in clients life; telephone support to clients or collaterals; social support services for locating, developing, or providing care services; or treatment planning session.\ CONSULTATION/EDUCATION SERVICES:Provides behavioral, emotional and/ or mental health and/or substance abuse consultation to community at large, area agencies, the professional community and/or schools as required. Consultation can be educational or evaluative in nature. Specific areas of consultation/education are based on specialists areas of expertise or interest. Provides liaison services between two sponsoring agencies to help coordinate services to clients.Referral to Emergency Services when a higher level of intervention is properly assessed required.
