    The Substance Abuse Counselor is responsible for the delivery of direct services in a substance abuse treatment setting as well as representing Outreach to outside agencies in a professional manner in order to increase referrals. This role is dually responsible for recruitment and engagement of clients in need of treatment, maintaining communication with outside referral sources and direct services, and assessed for treatment in the community.

    Job Scope:

  • Conducts assessments, intakes and participates in level of care determinations by gathering appropriate data, orienting client to his/her rights and responsibilities and program expectations.

  • Completes necessary administrative procedures for admission to treatment and, as appropriate, assists in development of treatment plan.

  • Facilitates admission/referral to other programs or agencies for clients in need of services not offered at Unit.

  • Provide individual, group, and/or family counseling services in a professional, ethical and competent manner.

  • Manages all aspects of assigned caseload during treatment, with special attention to appropriate levels of care and appropriate clinical interventions.

  • Provide backup coverage in the event of other clinical staff absences.

  • Complete all needed documentation and reports in a timely manner. Informs Program leadership (Director, Asst. Director, and/or Coordinator) of any problems or anticipated inability to meet deadlines.

  • Actively participate in staff meetings, case conferences and other unit meetings.

  • Develop and maintain positive relationships with referents and other collateral contacts (i.e., Probation, Parole, CPS, managed care, E.A.P.’s, courts, etc.) through timely written, phone, computerized or other responses.

  • Maintain communication and teamwork with clinical staff to integrate and enhance clinical services.

  • Conducts toxicology testing, as needed

  • Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.

  • Insure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan and personnel policy and procedure.

    Each role at Outreach has identified production and outcome goals. These goals may be stated as a number or as an outcome within an identified time frame. These goals are intended to provide each job holder the knowledge they need about expected performance. Accomplishments against these goals will be a discussion point in the annual evaluation.

    Qualifications

  • CASAC, CASAC-T, CRC, LMHC, LMSW, LMFT, LCSW or higher preferred.

  • Experience working in 822 clinics, as well as community outreach preferred

  • Ability to work effectively in collaboration with diverse groups of people

  • Flexibility in scheduling, if needed

    For this Long Island, NY position the expected based salary is $40,000.00 - $65,000.00/ year.
