Position Summary:

Provides clinical education and evaluation to nursing and patient care staff, and facilitates evidenced-based research within a specialized area of nursing. Collaborates with medical practitioners to incorporate nursing processes into the plan of care for a specialized group of patients. Performs direct specialty nursing services to patients as required, ensuring adherence to Joint Commission standards and all related policies, procedures, and guidelines.

Essential Functions and Responsibilities

:

  1. Provides educational leadership to patients and care providers to enhance specialized patient care within established clinical protocols.

  2. Assists patients and caregivers with educational needs, problem resolution, and health management across the continuum of care.

  3. Collaborates with medical providers, patient care staff, and unit management in the planning, implementation, and delivery of educational curricula.

  4. Initiates clinical skills development programs within the parameters of established clinical and preceptor models; monitors trends and implements educational strategies to ensure compliance with quality standards and parameters.

  5. Provides direct specialized nursing services to an assigned group of patients, providing specialized patient care within nursing protocols and assisting in the performance of all related nursing duties.

Liaison

Essential Functions & Responsibilities:

  1. First to assess patients, reviews their medical records and determines whether they should be admitted for care andthe appropriate level of care; may include admission/transfer reviews, utilization reviews, and surgical schedule reviews.

  2. Applies expert knowledge of payer authorization/certification to determinepatient’s eligibility for care andlevel of care options.

  3. Explains the types of care and services the facility offers to patients and their families.

Uses strong diplomatic skills to work directly with the physicians and hospital staff to coordinate the delivery of high quality, individualized, cost- effective patient care.

  1. Works with the facility to schedule therapy sessions and doctor visits; coordinates with other staff members to ensure smooth patient discharge.

Qualifications:

Required:

  • Nursing degree from an accredited educational institution.

  • State licensure as a registered nurse.

  • BLS certification.

  • Minimum Experience of three years in acute hospital setting

Preferred:

  • Bachelor's degree in nursing.

  • One year of teaching experience.

  • Utilization management (review) and discharge planning experience

  • Certification in Case Management, either CCM or ACM (Commission for Case Management or American Case Management Association)
