Premier Genesee, a 160-bed skilled nursing facility with an active 40-bed rehab unit is seeking a Director of Social Work to oversee Familiarity with care planning, admission processes, discharge planning, MOLST, resident rights is required and at least one-year experience working in skilled nursing setting.

Premier Genesee, a 160-bed skilled nursing facility with an active 40-bed rehab unit. Premier Genesee is seeking a Director of Social Work to oversee the long term care units at the facility, the departmental staff, and assure adherence to all current NYS and federal regulations. This position provides a wonderful opportunity to make a meaningful impact in the lives of our residents and their families!

The ideal candidate will have work experience in the social services field, within a long term care family.

Qualifications include:

Knowledge of MDS is essential Basic computer knowledge Knowledge of state regulation Long term care experience

Job Duties include:

Serve as an advocate for our residents and function as a liaison between the resident, family member, and staff. Review and follow up with any concerns Conduct initial, quarterly, and annual social work assessments and develop individualized care plans Oversee care plan meetings with interdisciplinary staff, residents, and their families Assisting with the admission and discharge process Providing resident with referrals as needed Knowledge of residents rights Completing MOLST forms Completes all tasks as assigned by administration Complete social work portions of MDS Adhere to DOH standards
