Early Childhood and Family Mental Health (ECFMH) Case Manager

Summary of Position: Under the supervision of the Early Childhood Programs Administrator and the clinical supervision of the Early Childhood and Family Mental Health (ECFMH) Clinical Administrator/ CYFS Senior Clinician, deliver ongoing community-based assessment, treatment and supports for young children (0-6 years) and their families experiencing or at-risk for severe emotional disturbance and their families in the community and within the New Leaf Family Childcare Center. This is a full-time benefit eligible position working on-site where hours are typically worked Monday – Friday 8a – 4:30pm.

Specific Duties and Responsibilities:

  • Provide support and supervision to therapeutic foster care providers, adoptive parents and / or biological parents.

  • Provide ongoing supports and services to children experiencing a severe emotional / behavioral disturbance and their families.

  • Participation and facilitation of ongoing treatment team meetings, participation in DCF Case Plan Reviews, and participation in educational meetings.

  • Develop and implement treatment plans, which provide for the delivery of services and    

  • interventions as identified by the treatment team.

  • Assist in developing natural supports for children and youth within their communities.

  • Work collaboratively with local DCF office, schools and other agencies providing services to children and youth experiencing a severe emotional / behavioral disturbance and their families.

  • Complete all assigned paperwork accurately and with the identified timeframe utilizing an electronic medical record. Maintain client records to include writing monthly summaries, treatment team summaries and all other required paperwork relevant to providing client services.

  • Provide crisis intervention and support to children experiencing a severe emotional/ behavioral disturbance and their families.

  • Provide client transportation as needed in your own vehicle to and from community activities, foster placements, mental health appointments, etc.  Staff is responsible to carry adequate auto insurance coverage.

  • To familiarize, implement and comply with WCMHS policies and procedures.

  • Follow supervision directives. 

  • Perform other duties / responsibilities as assigned by supervisor.

    Qualifications: 

  • BA in human services or related field required.  

  • Two years of human service delivery with children and families, particularly with children experiencing moderate to severe behavioral and mental health challenges preferred. 

  • Excellent verbal and written communication skills.

  • Excellent time management required.

  • Able to move around childcare and community settings without accommodation.

  • Able to lift and move up to 50 pounds.

  • Able to walk and/or run around with children up to one mile in one continuous session without accommodation.

  • Able to move to a sitting position on the floor and back to a standing position without accommodation.

  • Able to drive clients and family members in own personal vehicle as needed.

  • Possession of a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle are required.
