Join our Team! Penquis, a social services agency serving Penobscot, Piscataquis, and Knox counties for over 50 years, is currently seeking a Full Time (40 hours per week), Health Home Coordinator. As the Health Home Coordinator, you will provide care coordination/case management to adults who have an approved mental health diagnosis. This position is remote, with travel and in-person meetings required in the Dover-Foxcroft and Bangor areas depending on the needs of the program.

Location: Bangor, Maine

Health Home Coordinator Responsibilities:

  • Work with individuals to address behavioral health needs, as well as integrating physical health and medical needs.

  • Participate in a team-based approach to care by working in collaboration with your supervisor, Peer Support Specialist, Clinician, Nurse Care Manager, Psychiatric and Medical Consultants.

  • Provide accurate clinical assessments and annual assessments.

  • Independently track all Mainecare review dates, assessment reviews, and treatment plan due dates for a year at a time.

  • Participate in ensuring the delivery of crisis intervention and resolution services, providing follow-up services to ensure that a crisis is resolved and assists in the development and implementation of crisis plans.

Health Home Coordinator Requirements:

  • High School Diploma, and full or conditional MHRT-C certification.

  • 2-3 years’ experience working with children, adolescents or adults with disabilities.

  • Must have a valid State of Maine driver’s license in good standing and reliable insured transportation and available to work flexible hours to meet the needs of the client and families.

  • Upon hire and periodically thereafter, must comply with Penquis background check requirements with satisfactory results as outlined in current contract provisions.

The rate of pay for this position starts at $20.95 per hour, and additional compensation may be given consideration with relevant experience.
