University of Maryland, Baltimore Community Program Coordinator in Baltimore, Maryland
The UMB, Community Engagement Center Baltimore School of Nursingis seeking a hourly contract Community Program Coordinator. TheCommunity Outreach Worker (COW)is responsible for helping neighbors navigate and access services. The COW will refer the neighbors to other members of the West Baltimore RICH Collaborative team for follow up with social and economic issues and increase awareness and need for health prevention related to hypertension and social isolation. This position also participates in outreach and health screening activities organized by the West Baltimore RICH Collaborative. Contacts and interactions vary and may involve multiple constituencies such as direct interaction with the West Baltimore RICH executive management, community organizers, the general public, patients, physicians, colleagues, assigned staff, vendors, contractors and consultants for the purpose of providing and exchanging information. The ideal candidate will personally connect with the Administration and Finance Guiding Principles which closely mirror UMB’s core values: Respect and Integrity, Well-Being and Sustainability, Equity and Justice, and Innovation and Discovery. * *Essential Functions: * Establishes a supportive relationship with neighbors and their families to encourage their active engagement in their health care and support programs. Recruit program participants. * Assists with scheduling appointments as needed and assists neighbors to find appropriate resources. * Collaborates with care team or partners to help neighbors understand care plans and self-care management goals while trying to reduce cultural and socio-economic barriers. * Provides timely communication to the team and reports neighbor needs/issues to ensure coordination of care and appropriate referrals to other members of the care team. * Assists neighbors in completing applications or other requirements for accessing services and follows up to provide assistance to support neighbors accessing services. * Participates in focused outreach, community health screenings and community events to promotes services and programs within the West Baltimore RICH Collaborative. * Engages in community-based events which occasionally occur in the evening hours and on weekends. * Provides referrals for community services as appropriate and in consultation with the care team. * Provides accurate and timely documentation and reports in compliance with UMB, partner sites and funder needs. * Manages data collection and records for site, partner and funder reports. * Coordinates, monitors, and/or supervises and students and/or volunteers in outreach events and activities. * Develops, schedule, market and execute outreach program and events. * Interacts and maintain liaison with students, staff, faculty and outside partners about program and neighbors needs. * Builds and maintain strong relationships with community members and partners to grow programming. * Other duties as assigned. Education:Bachelor's degree Experience:1 year administrative or program coordination experience. Preferred:Community health worker training and/or experience; outreach experience; Previous job experience in health care or social services. Required Knowledge/Skills/Abilities: Knowledge of the UMB, West Baltimore RICH Collaborative and practice-site operations, operating principles, guidelines and bylaws. Excellent leadership, customer service, organizational and presentation skills as well as the ability to effectively communicate the West Baltimore RICH Collaborative vision, and motivate others to achieve it organizationally, departmentally, and personally/professionally. Ability to communicate effectively (verbally and in writing). Ability to plan and organize work initiatives to successfully accomplish center/organizational goals and objectives. Ability to multi-task, prioritize and delegate as appropriate. Strong analytical, problem solving and interpersonal skills. Ability to identify, develop and implement short/long-term strategic goals and objectives. Ability to develop and maintain customer relationships; influence, build credibility and trust. Ability to think critically as well as apply critical thinking skills. Ability to advocate for patients/clients. Ability to document in using an electronic health record. Physical Demands: Must have the use of sensory skills to effectively communicate and interact with other employees and the public. Ability to effectively use and operate office related equipment, such as, but not limited to personal computer, calculator, copier, and fax machine. Ability to lift, carry, push or pull heavy objects upwards of 75lbs as well as squat, walk, climb, bend, crouch, stoop, kneel, stand, grasp, reach, pull and repetitive motions.
