Care Transition Manager / Social Worker, LMSW / Part Time

Work location: Texas Health Azle, 108 Denver Trail Azle TX 76020

Work hours: Part time, 24 hours weekly (benefited), Days, Wednesday through Saturday, at least 3 of the 4 weekends each month (3 to 4 weekend day shifts a month)

Texas Health Azle is a 36-bed, community-based hospital serving the tri-county area since the 1950s. We provide exceptional care to the suburban areas of Azle, Lake Worth, Springtown and surrounding areas.

We’re accredited through The Joint Commission for disease specific care of Chest Pain patients and we are accredited through the Texas Department of Health & Human Services as a Level IV Support Stroke facility, a Level IV Trauma Center and a Pathways to Excellence-designated hospital. We specialize in outpatient surgery, rehabilitative physical and hand therapy, and advanced imaging like mammography and CT scanning. Here’s What You Need

· Master's Degree Social Work is required. (Individuals hired as a CTSW prior to May 11, 2017 will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017)

· Three (3) years in hospital/medical social work is highly preferred.

· One (1) year of discharge planning/care management is preferred.

· LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire.

· CPR - Cardiopulmonary Resuscitation is required upon hire.

· ACM - Accredited Case Manager is preferred upon hire or

· CCM - Certified Case Manager is preferred upon hire.

· ANCC certifications also preferred.

What You Will Do

· Facilitates care conferences for complex transitions, placement and palliative care needs.

Serves as a point of contact for all identified stakeholders.

· Proactively identifies patients who no longer meet continued stay criteria and communicates with the physician team. Attempts to schedule PCP, specialist or clinic follow up appointments for patients.
