UCHealth Care Coordinator in Aurora, Colorado

Summary:

The ideal candidate should possess excellent problem-solving skills, the ability to manage conflicts with external vendors, and the capability to handle physician and patient complaints with grace and professionalism. This person should also bring ideas for continuous improvement, be dedicated to working as part of a team, and be self-sufficient when working independently.

At the direction of professional Care Management or Social Work staff, coordinates transitions of care, conducts and documents socio-economic review and screening to support the coordination of care.

Work Schedule: Full Time Days

Responsibilities:

  • Coordinates with outside agencies, post-acute care or specialty facilities, transportation agencies, and others as indicated. Assists clients and patients in connecting with appropriate Physicians, insurers, Payer programs, and other resources to meet social and clinical needs.

  • Collaborates with the care management and behavioral health teams for effective patient or client outreach, care coordination and hand-overs across the continuum of care. Refers patients and provides educational resources to patients per protocols.

  • Conducts scripted phone calls and accesses systems to retrieve patient information for clinical care team and send appropriate information for placement or transfer. Retrieves record, imaging and diagnostic results, pathology and other requests for treatment teams.

  • Arranges care conferences as needed. Supports Inpatient and Ambulatory multidisciplinary team meetings as assigned. Works with internal and external resources to coordinate placement, discharge planning, case management, behavioral health and follow-up care.

  • Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action.

Requirements:

  • High School diploma or GED.

    The pay range for this position is: $19.67 - $25.57 / hour. Pay is dependent on applicant's relevant experience.
