Job Title: Resident Social Services Coordinator

Job Type: Part-time (20-25 hours per week)

Job Summary:

We are seeking a Resident Social Services Coordinator to provide general case management, intake, and referral services to residents in need of assistance. The successful candidate will be responsible for evaluating the health, psychological, and social needs of residents, developing individually tailored case plans for services, and periodically reassessing the resident's situation and needs. The Resident Social Services Coordinator will also establish linkages with agencies and service providers in the community and determine/develop the best price for services in personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurses, and preventative health.

Responsibilities:

  • Provide general case management, intake, and referral services to residents in need of assistance

  • Evaluate the health, psychological, and social needs of residents

  • Develop individually tailored case plans for services

  • Periodically reassess the resident's situation and needs

  • Establish linkages with agencies and service providers in the community

Requirements:

  • Minimum of 2 years of experience in case management or related field

  • Knowledge of community resources and services

  • Excellent communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Strong organizational and time management skills
