This Health Advocate role is a Hybrid role. You will be required to travel to facilities a couple times a week depending on the needs for the business.

Normal Working Hours: Monday through Friday 8am – 4:30pm EST. Occasional early/evening shifts per the needs of the team.

This position offers Emory and Aetna an opportunity to work together to support Emory’s culture of well-being. This position involves initiative planning, ongoing client relationship management and satisfaction with assigned initiatives that impact the delivery of products and services to the internal and external environment. This role will collaborate with external vendors, external consultants, consulting team members, and/or matrixed staff as required to support initiatives. Further, this role will transfer knowledge, best practices (External and internal), methodology, and tools to client organizations. They will participate as a team member, may be responsible for concurrent initiatives or act as a team lead for less complex projects based on resource requirements. Onsite Nurse Advocate

  • The Dedicated on-site Nurse Advocate for Emory University and Emory Healthcare will engage, educate, promote, and influence members to enable decisions related to achieving and maintaining optimal health status.

  • She/he will work with both members, EHN clinical transformation, and the HR Benefits /Well-being teams on ways to deliver the best member experience and optimize the benefits offered through the employee benefits plan.

  • Collaborate with the Aetna Account team, and Emory HR Benefits/Well-being team to provide recommendations on future benefit enhancements

  • Become a Subject Matter Expert on HR Benefit/Well-being programs offered to Emory Employees & their families so that they can be used to counsel and coach to incent additional engagement. Provide recommendations around targeted education and communication to specific employees needing additional support. Organizations are increasingly recognizing the importance of addressing Social Determinants of Health (SDOH) to support the well-being of their employees. SDOH, such as access to healthcare, transportation, housing, education, and healthy food, directly impacts employee productivity, engagement, and overall health outcomes.

  • Work collaboratively with Aetna’s Clinical Program Consultant to review Population Health, High-Cost Claimant, ER “frequent Fliers” reporting and identify members for additional and/or targeted outreach.

  • Support clinical integration and collaboration on behalf of the member in conjunction with the Aetna One Choice nurse and EHN clinicians.

  • Assist members in identifying modifiable risk factors, referring to and leveraging Emory's coaching resources to coach on lifestyle change, including weight management, quitting tobacco, stress reduction, nutrition, and increasing physical activity.

  • Under the direction of the AVP of Health Management and Corp Director of Occupational Health Services & Wellness, work to deliver a preventive well-being program targeting employees who:

  • Have been diagnosed with pre-hypertension/hypertension.

  • Are at risk for or diagnosed with musculoskeletal challenges.

  • Open to 1:1 coaching to address lifestyle risks and optimize health.

  • Act as a resource for referrals from Emory’s Occupational Health Services team, Health and Well-being team, and others to address employees’ health-related issues and concerns.

In addition to they will provided face-to-face coaching, the Onsite Health Advocate seeks to engage employees at the various Emory locations through:

  • Group coaching sessions example: Lunch & Learn, Condition Informational Sessions

  • Discuss lifestyle choices and their unique role in healthcare outcomes.

  • Discuss the prevention and associated cost burden of lifestyle-related diseases, such as hyperlipidemia, cardiovascular disease, prediabetes, diabetes, hypertension, obesity, and cancer.

  • Apply nutritional principles based on science that shows how nutrition can address the pathophysiology of many chronic diseases, including inflammation, glucose control, gastrointestinal health.

  • Discuss the pathophysiology between physical activity components and health outcomes.

  • Describe the benefits of physical activity in preventing or treating disease in healthy older adults, pregnant women, children and adolescents, persons with obesity or disability, cardiovascular disease, diabetes, cancer, disability, and stroke.

  • Describe sleep’s role in health and chronic disease pathophysiology.

  • Summarize lifestyle-based interventions that can improve sleep health.

  • Describe the health effects of tobacco, alcohol, and other frequently used substances and the benefits of cessation.

  • Apply screening tools for stress, depression, and anxiety in clinical practice.

  • Explain the relationship and pathophysiology between emotional and physical health.

  • Summarize the nature of stress, and the role of stressors, such as adverse child experiences, and identify manageable elements of pathogenic stress reactions via

  • Telephone and email follow-ups,

  • Health education seminars using Emory-appropriate materials, Health awareness tables, Team meetings, meet-and-greet sessions, Health education newsletters

  • Other onsite events coordinated by the employer group

  • Very high level of customer service

Required Qualifications

  • Must have active and unrestricted RN licensure in the state of Georgia

  • 3+ years of data interpretation and analysis experience.

  • 3+ years of project management experience.

  • 3+ years of Demonstrated leadership

Preferred Qualifications

  • Case Manager Experience

  • Coaching and Education Experience

  • Demonstrated ability to effectively coordinate multiple projects simultaneously.

  • Excellent verbal and written communication skills.

  • Certified Case Manager (CCM) certification

Education

Bachelor’s Degree or equivalent work experience

Pay Range

The typical pay range for this role is:

$54,095.00 - $116,760.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
