Position Summary

The community health worker will help promote, maintain, and improve the health of patients through health screenings, informal counseling, and self-management education programs. The Community Health worker will assist with connecting support programs to a broad range of individuals including persons experiencing chronic medical conditions, mental illness and substance abuse issues. The candidate must be an excellent communicator and collaborate with clinicians from medical clinics to help the patient meet his/her goals. These treatments are provided in accordance with best practice, professional ethics, and boundaries.

Minimum Qualifications

  • Associate’s Degree or two (2) years of college from a regionally accredited college or university OR completion of a certified community health worker training

  • Minimum of one (1) year of clerical experience in community health program to include experience utilizing a personal computer.

  • Resident of the City of East Point and surrounding areas and have a valid Georgia’s driver license

  • Great communication skills

  • Must have a reliable source of transportation.

    Preferred Qualifications

  • Two (2) years of community engagement experience

Job Duties

Description of Job Duty

  • Offers assistance and functions as a “guide” for patients needing healthcare.

  • Helps patients move through the complex the healthcare system – getting them more timely treatment and more information about treatment options.

  • Serves as a person for the patient to lean on for advice, support and direction. A navigator is someone who understands the patient’s fears and hopes, and who removes barriers to effective care by coordinating services, increasing a cancer patient’s chances for survival and quality of life.

  • Provides an environment conducive to general learning by using the essential helper characteristics (empathy, respect, warmth, genuineness, immediacy and concreteness) and by being responsive to the individualized needs of others.

  • Encourages others to empowerment by listening with warmth and respect, supporting whatever decisions are made, genuinely caring about the lives of the participants, and interacting in a concrete, sensitive and non-judgmental fashion to help ensure a successful outcome of whatever the situation may be.

  • Facilitate change by providing a supportive environment, information or other resources; this makes Navigators an ideal source of assistance for motivating others to make health related behavior changes.

  • Serves as a contact person for patients, their families and staff to coordinate communication, scheduling, patient information and other activities of care between patients and multidisciplinary providers.

  • Works closely with coordination teams to better schedule patients to the appropriate service with a focus on patient convenience and timeliness of service.

  • Collaborates with the scheduling teams of the various disciplines within the health system to ensure seamless transitions between inpatient and outpatient care and between multidisciplinary providers.

  • Guides patients on treatment activity and next steps in appointments (i.e. surgical prep, imaging, labs, tests, which provider to see next, etc.).

  • Assists in managing the transitions of care across care settings, ensuring optimal communication and planning between care providers across different settings

Immunization Requirements

It is MSM’s Immunization policy that all Prospective Employees are required to provide proof that they are vaccinated against COVID -19 prior to commencement of employment. If employment will commence during Flu Season, MSM requires all individuals to provide proof that they are vaccinated against Seasonal Influenza for the current Flu Season, unless granted an exemption. Failure to provide proof of vaccination for any required vaccines or obtain an exemption from MSM will result in rescission of a candidate’s offer of employment or disciplinary action up to and including termination.
