Trinity Health Case Manager (PATH) in Atlanta, Georgia

Description:

Position Purpose

As a PATH Case Manager at Mercy Care, you will work with individuals experiencing homelessness who also have severe mental health diagnoses. Your role is to actively engage with our target populations, offering support in establishing and achieving their goals around the social determinants of health (SDOH). You will be responsible for linking clients to community resources, mental health services, and housing assistance, improving their overall health outcomes.

What You Will Do

  • Conduct assessments, intakes, and case management for clients with mental health needs.

  • Assist clients for up to 120 days, connecting them to mental health and housing resources.

  • Collaborate with clinical teams to ensure clients receive appropriate care.

  • Participate in homeless coalition meetings and case conferencing.

  • Maintain accurate client records in the Homeless Management Information System (HMIS) and other grant-prescribed data systems.

Minimum Qualifications

  • Bachelor's Degree required; minimum two years of college coursework in a related human services field.

  • At least 1 year of case management experience with mental health or substance abuse populations.

  • Ability to work independently and within a team, meeting time-sensitive deadlines.

  • Flexibility to travel between Mercy Care PATH sites.

  • Active Georgia Driver’s License.
