The Community Health Worker (CHW) provides support to and builds relationships with patients who have a substance use /chemical dependency diagnosis to ensure a

successful transition from ED/ OB-ED in hospital to the community and to reduce re-admissions to the ED/hospital. Support includes brief assessment of status of substance use /chemical dependency issue, assessment of readiness for change, identification of coverage for services, referral to appropriate services and follow-up to ensure services are received. Serves as a link between organizations serving persons with chemical dependency needs and the Medical Center to improve coordination of services.

Required qualifications:

• 3 years experience working with substance use disorder/chemical dependency adult clients.

• Within 6 months of hire California Certified Alcohol Drug Counselor I - California Consortium of Addiction Programs and Professionals.

Preferred qualifications:

• Completion of CADC or CATC program at hire

• Bachelor's Degree in Human Services, Social Work, Marriage & Family Therapy, Psychology, or related field.
