Sutter Health Float Pool Staff Nurse, Medical/Surgical/Telemetry-- in Antioch, California

Position Overview:

The primary role of the Staff II RN is to coordinate the delivery of care for the patient population they serve from admission through discharge. Included in this delivery of care are all aspects of the physical, psychosocial, safety and educational needs of the patient and their family/support system. This function is performed in collaboration with an interdisciplinary team including but not limited to nursing peers, attending physicians and ancillary support services. The staff RN uses the nursing process, medical therapeutics, technology and personal interventions to accomplish this.

JOB ACCOUNTABILITIES

Patient Care Process

• Provides non-judgmental care towards all patients, regardless of lifestyle, race, religion or beliefs

• Reviews all available assessment data and incorporates into overall assessment of the patient

• Assesses patients clinical status and accurately documents in the EHR in a timely manner

• Documents care provided to patient in the EHR in real time

• Demonstrates early recognition of potential and developing problems.

• Evaluates and documents effectiveness of interventions in the EHR

• Initiates Nursing Care Plan upon admission and updates every shift.

• Initiates patient Education upon admission and updates every shift

• Completes required patient screenings within time frame allowed

• Identifies Core Measure patients and ensures completion of all elements

• Notifies MD timely using SBAR format and documents communication in the EHR

Leadership / Teamwork

• Is able to carry a full patient assignment and meet the needs of each individual patient

• Is able to anticipate and handle unexpected situations

• Manages multiple priorities systematically, keeping team members informed of the plan

• Is self-directed, resolving minor problems on own and if unable utilizes chain of command

• Fosters a positive attitude towards department and facility. Shows awareness of impact of own behavior on others

• Communicates changes in patient condition to management as needed

• Deals with interpersonal conflicts directly and in private, reporting irreconcilable differences and unprofessional behavior to Nursing Director / Manager in an appropriate and timely manner

• Initiates activities when patient census is down, stocking rooms, cleaning up different areas within the unit

• Demonstrate support for team members, to ensure the team accomplishes goals. Exemplifies team loyalty

• Seeks out constructive feedback. Shares opinions with respect

• Problem solves independently at their level, escalates when necessary

Clinical Technical Skills

• Demonstrates ability to operate all equipment required to perform job safely, per manufacture guidelines and hospital policy

• Maintains a safe, uncluttered working environment (patient rooms, nursing station) ensures all egresses are easily accessible

• Maintains department specific skills

• Utilizes patient mobilization equipment

• Completes all trainings on time

Communication

• Communicates patients plan of care to co-workers and on-coming/off-going shift and between departments in a positive manner

• Establishes positive rapport with patients, families/support systems, physicians and co-workers

• Uses SBAR format for patient handoffs and utilizes communication tools as patients move throughout facility for care

• Utilizes closed loop communication to ensure messages are received and acknowledged to ensure safe patient care

Professional Growth & Development

• Accepts responsibility for own actions and takes a proactive approach to problem resolution

• Takes responsibility to become familiar with new process or procedures and to get checked off within required time frame

• Acknowledges own limitations and seeks help when needed.

• Identifies own learning needs and follows through to gain needed knowledge and experience

• Seeks out and obtains clinical and management learning opportunities

• Continuing education is related to specific area of practice and documented in employee file

• Working towards staff nurse 3 , specialty certification, higher education (BSN or MSN) to increase knowledge base

• Works towards ensuring goals set in prior evaluation are met

EDUCATION

Equivalent experience will be accepted in lieu of the required degree or diploma.

Associate's: Graduate of accredited school of Nursing

Bachelor's: B.S.N.

CERTIFICATION & LICENSURE

RN-Registered Nurse of California Upon Hire

BLS-Basic Life Support Healthcare Provider Upon Hire

Sutter Delta Medical Center, Department: Telemetry, • 2 years of basic Medical / Surgical acute care nursing experience or at least one year of experience in specialty area.

• Experience in Cardiac and Telemetry nursing in an acute care setting and meet all required certification of the position.

• Experience that allows the assessment, treatment and management of conditions that affect the complex cardiovascular system.

• Experience providing care to patients across the lifespan, from adolescent to the elderly.

SKILLS AND KNOWLEDGE

Must demonstrate the following:

• basic Medical / Surgical nursing skills,

• basic competencies in the nursing process and pharmacology,

• basic knowledge of adolescent,

• adult and geriatric population,

• ability to work with all levels of nursing personnel and assist them in a resource capacity,

• effective English written and verbal communication skills.

Pay Range is $72.21 to $97.72 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
