Amergis RN Hospice Case Manager in Annapolis, Maryland

The Registered Nurse- Hospice Case Manager is responsible for providing care, comfort and supportto terminally ill patients. TheRegistered Nurse - Hospice assumes the responsibility and accountability forthe delivery of hospice patient care in various settings. The Registered Nurse demonstrates the abilityto make clinical judgments in an effective and efficient manner withsupervision. The Registered Nurse willalso demonstrate critical thinking and performance ability in the coordinationof patient care. Consistently performsaccording to nursing standards and is accountable in managing patient care andassisting others in the management of patient care.

Position Details:

  • Location: Prince George's County and Anne Arundel County

  • Mainly working within the Prince George’s County territory but will sometimes float to Anne Arundel County.

  • Schedule: Monday-Friday, 8a-5p, 40 hours/week

  • No weekends, No-on-call

  • Start Date: ASAP

  • Contract Length: 13 Weeks (opportunity for extension)

  • Local Pay: $60/hour

  • Mileage Reimbursement : 67 cents per mile

  • Caseload & number of visits per day:

  • Caseload: 15-20 patients

  • Roughly 5 visits per day

    Essential Duties andResponsibilities:

  • Will be operating in both a Case Manager Capacity and Hands-on Hospice care (Plan of care, order supplies and medication, etc.)

  • Will be tasked in visiting hospice patients at various locations (patients homes, LTC/Assisted Living Facilities, etc.)

  • Ordering meds/supplies

  • Will have to drive own vehicle (mileage reimbursed)

  • Will not drive patients

  • Completes initial assessment of patient and family/caregivers to ascertain Hospice eligibility and needs - including physical, emotional, spiritual and mental status.

  • Prepares and implements the initial care plan for patient/family unit with Medical Director approval.

  • Provides direct nursing care to patients in order to maintain highest level of care.

  • Inform Team Leader of utilization of other team members, outside agencies, and community resources upon initial assessment findings.

  • Document clinical findings to assure initial hospice eligibility and participate in discharge planning when necessary working with relevant patient care, social services, and other departments

  • Teaches family members or hired caregivers, patient care skills, including: pain control, medication (RN titrates medication within prescribed range), injections, draws blood for labs, manages IV's, dressing changes, hygiene, nutrition, checking vital signs in accordance with orders from physician

  • Ensures that arrangements for equipment necessary in the home setting are made available.

  • Attends to patient and family at time of patient’s death if it occurs at the time of visit.

  • Participates in Admissions department projects and process improvement strategies.

  • Establishes and maintains effective working relationships with all possible Hospice referral sources in the service area. Meet with healthcare administrators representing Gilchrist Hospice Care within the community.

  • Obtain necessary consents in accordance with applicable regulations & reimbursement policies.

    Minimum Requirements:

  • Current Maryland or Compact RN License

  • Complies with all relevant professional standards of practice

  • Participation and completion of Amergis' Competency program when applicable

  • Current CPR if applicable

  • TB questionnaire, PPD or chest x-ray if applicable

  • Current Health certificate (per contract or state regulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable to job site

  • Understand patient confidentiality and HIPAA requirements

  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

  • Computer proficiency required

  • Must be at least 18 years of age
