Essential duties and responsibilities:
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
- Serves as the program administrator for the PATH (Projects for Assistance in Transition from Homelessness) Program
- Responsible for the state and federal reporting requirements for the PATH program
- Responsible for service delivery for specialized population for adults with behavioral health issues and experiencing homelessness
- Services include outreach, referral to community resources, case management, substance abuse treatment, housing and/or employment skills training
- Works closely with other community partners serving the homeless through the community Continuum of Care (COC) efforts to coordinate care between agencies and programs to engage individuals that may not otherwise receive services
- Will be trained in state-approved curriculums that emphasize evidence-based practices to help improve daily functioning
- Organize and provide culturally competent care activities and collaboration among all participants involved with the individual's care
- Additional duties can include: completing assessment and planning, working within the team to ensure appropriate services are delivered, engaging individuals by taking a person-centered approach to help identify goals for services/treatment
- Assist and collaborate with the interdisciplinary Behavioral Health Team in promoting skills development and community stability
- Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised
Required qualifications to be successful in the job:
- Bachelor's degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialing
- Minimum of three (3) years of experience working in a behavioral health or human services related field
- Graduate work in a related field may be substituted for up to one (1) year of experience
- Must have a valid Texas driver's license and be insurable under agency vehicle policy
- Must be 18 years of age or older
Preferred qualifications to be successful in the job:
- Bilingual
- Previous experience working with individuals experiencing homelessness
