Schedule: Hourly, Full-Time

Shift : Monday-Friday with Rotating Supervisor On-Call.

Location: Hospice is based out of our Altamonte Springs Office.

• The role you’ll contribute: The Patient Care Manager is responsible for the management of multiple team(s) of clinical and support staff providing hospice and palliative care to an assigned case load of patients. The position is responsible for the overall direction, coordination, and evaluation of all staff on their assigned team. The value you’ll bring to the team:

• Promotes and markets Hospice of the Comforter in the community

• EDUCATION AND EXPERIENCE REQUIRED:· Associates Degree. · Minimum of 3 years’ experience as a Registered Nurse including three years supervisory experience. · An equivalent combination of education and experience may be substituted at the discretion of the Administrator.EDUCATION AND EXPERIENCE PREFERRED:· Bachelor of Science in Nursing, Business Administration, Healthcare Management, or related field.· Home health and/or acute care nursing experience.LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:· A clear/active Florida Registered Nurse license.· Basic Life Support Certification. · Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.· Maintain valid personal automobile insurance.LICENSURE, CERTIFICATION OR REGISTRATION PREFERRE
