We are looking for a Transition Coordinator to join our team!

Overview: Join our team at the Albany Langan School and Prospect Center where you will play a pivotal role in shaping the futures of our students. As the Transition Coordinator, you will lead the implementation and evaluation of our Vocational Education and LIFE Skills program. This program is designed to empower students with severe disabilities by fostering independence and preparing them for meaningful adult lives.

As Transition Coordinator, you will:

  • Implement and evaluate the Vocational Education and LIFE Skills program.

  • Facilitate career skill and interest inventories, conduct career exploration activities, and provide vocational programming for eligible students.

  • Collaborate closely with teachers to ensure effective transition programming.

  • Mentor and supervise the Transition Specialist Level I.

  • Identify and develop external placement opportunities for students, ensuring meaningful work experiences.

  • Participate actively in transition planning meetings with students, families, and school teams.

  • Coordinate school and community-based work experiences, evaluating student job performance according to IEP objectives.

  • Maintain relationships with businesses and agencies offering post-secondary services for students with disabilities.

    To excel in this role, you must possess:

  • A graduate degree (Masters or Doctorate) in Special Education.

  • Permanent NYS certification as a Special Education Teacher.

  • Specialized training in Level I and II vocational/traditional assessments and vocational skill development in students with severe disabilities.

  • At least 5 years of experience in teaching or supervising in special education, with a focus on secondary-age students with severe disabilities.

  • At least 3 years of experience in Vocational Education and/or life skills management with severely disabled students

    Required Skills:

  • Excellent communication skills, both verbal and written, capable of engaging diverse audiences.

  • Proficiency in using electronic tools for documentation, data management, and presentations.

  • Strong analytical skills, including the ability to interpret statistical data and make informed recommendations.

  • In-depth knowledge of special education laws, regulations, and curricula.

  • Ability to make independent judgments and apply program policies effectively.

  • Ability to lift up to 50lbs, position, and handle students with physical disabilities or behavior management needs.

    Additional Information:

  • This position requires a current, valid NYS Driver’s License in good standing.

  • Flexibility in work locations based on program needs.

    The pay range for this position is $69,400 - $73,554 and is commensurate with experience.
