Responsible for outreach activities, coordination of services of families, and assistance with specific health care, social, educational and employment activities and referrals. Organizes schedule with the supervisor to accommodate needs of families and program requirements. Work assignments are directed by the Senior Public Health Aide. Does related work as required. MINIMUM QUALIFICATIONS:Possession of a high school diploma or general equivalency diploma.SPECIAL REQUIREMENT:Possession of a valid New York State Driver’s License at the time of appointment and throughout employment
