Description

As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.

Responsibilities:

  • Coordinates, monitors, and documents the health care of foster children placed within the foster care program, including but are not limited to:

  • Tracking health and dental exams.

  • Scheduling and assisting with psychiatric med reviews for children in the FC program

  • Reviews and documents medical and dental reports and coordinates follow-up as directed

  • Coordinates referrals made to specialty clinics and follows up on recommended tests, treatments, or therapies

  • Follows up regarding emergency room visits and sick visits to ensure that the prescribed medical regimen is being carried out

  • On a case by case basis, completes or coordinates completion of forms required for specific purposes, i.e. school, working papers, or camp

  • Monitors both medical and dental compliance and notifies foster parents and foster care case manager when compliance is in jeopardy

  • Reviews completed examinations, medical consults, and med-ex forms for foster care children and electronically stores as an attachment into chart

  • Serves as a liaison between internal and external medical service consultants, foster care supervisor, and foster care case managers

  • Serves as a resource to program staff concerning the health status of children in the foster care program, specifically those children with chronic health needs

  • Participates in service planning and treatment reviews and clinical meetings as indicated

  • Maintains a close working relationship with community-based health care facilities. On a regular basis seek consults and lab work for identified children in the foster care program, reviews bills to ensure accuracy related to service provision, discusses and resolves any problems and obstacles to children accessing appropriate care

  • Travel to foster parent homes with the occasional transport is required for this position

Requirements and Education:

  • LPN Licensed in New York State

  • 1 to 2 years of experience, Experience working with youth and adolescent population desired

  • Valid NYS drivers license
