SUMMARY The Case Manager is responsible for assessing patients, creating treatment plans, and connecting patients to relevant patient navigation, case management, and behavioral health services. These case management services may include reducing barriers to accessing abortion care, LGBTQIA services, transportation for healthcare, housing/lodging, etc. The Case Manager works collaboratively with physicians, other clinicians, ancillary staff, and leadership throughout the agency. This position serves as a case management resource for other licensed staff throughout the agency. ESSENTIAL DUTIES and RESPONSIBILITIES include the following. Other duties may be assigned. Assessment and Coordination: Obtains a comprehensive relevant biopsychosocial history. Formulates a diagnosis/differentials based on the mental health assessment. Develops and implements treatment plans and implements interventions in accordance with care paths, protocols, MS&Gs and current evidence-based guidelines. Documents all patient information in EPIC within 72 hours. Provides patient education to facilitate optimal patient outcomes. Consults with other licensed providers as needed. Refers patients when care needs are beyond the scope of practice and knowledge proficiency. Patient Navigation and Case Management: Work with the patient to identify logistical needs to access care and linking the patient to appropriate resources as needed. Acts as an advocate for patients in the GAC program. Provides patients with resources and assists with gender-marker change letters and surgery referral letters. Acts as an information resource for abortion patients. Discusses visit expectations, costs, and answers any non-medical questions. Provides patients with additional education as needed around the selected abortion procedure based on LMP or sedation options. Is knowledgeable of all services provided at PPGOH, including but not limited to abortion education, gender-affirming care, primary care, etc. Facilitate communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care. Accommodate interpretive language services when needed. Proactively resolve barriers, support and/or escalate tough patient cases through professional communication between managers, providers and patients. Monitors and ensures compliance with OSHA, CLIA, ODH and state laws, policies and regulations. Advances own professional development through self-directed learning, continuing education, and participation in peer review for teams' evaluation.
