Job Description:

Join Our Team as an After Hours Hospice RN!

Schedule: 7 Days On, 7 Days Off (Work One Week, Take the Next Off)

  • Monday-Thursday: 5:00 PM - 8:00 AM

  • Friday: 5:00 PM - Monday 8:00 AM

On an Average day, you will:

  • Patient Assessment : Evaluate hospice patients to identify their physical, psychosocial, and environmental needs.

  • Care Planning : Make initial evaluations and re-evaluate nursing needs during each visit.

  • Communication: Report significant findings, problems, and changes in health conditions to the clinical supervisor, physician, and other care personnel.

  • Patient Counseling : Provide safety plans and utilize family and community resources forpatient care.

  • Care Implementation: Ensure quality and continuity of care for an assigned case load to achieve patient outcomes.

  • Action Initiation: Identify and act on deviations from normal nursing and patient condition statuses.

  • Plan Verification: Verify the Plan of Care before each visit and deliver care according to physician’s orders, assessment data, and established guidelines.

Requirements:

  • Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School ofNursing.

  • 1+ year of experience as a nurse in aclinical care setting.

  • Home Health experience is preferred, but not required.

  • Current State license as a Registered Nurse.

  • Ability to travel approximately 50%

  • Excellent verbal and written communication skills.

  • Preferred to live in the service area
