    Qualified LPN candidates will have:

  • Graduate of a state approved school of nursing

  • A current license as a Licensed Practical Nurse in New York State

  • CPR certification.

    Role Responsibilities:

  • Administers medications and performs treatments as ordered by physician. Documents appropriately.

  • Assists with direct resident care as necessary, including personal care, feeding, care of environment changes, and safety.

  • Ensures recording of Intakes and Output, toileting records, turning and positioning sheets and other documentation in neighborhood.

  • Completes incident reports and, initiates investigations occurring on assigned unit, and forwards to RN nurse manager.

  • Notifies RN for assessment as needed.

  • Initiates the transfer of residents to the hospital as necessary and ensures completion of necessary paperwork.
